Hello doctor,

I am writing because my elder brother, at the age of 50 and very physically active, was just found to have stage 4 lung cancer. My elder brother has always been an avid hiker and treks once a month. He has never smoked a day in his life. Our whole family is quite perplexed at how this is possible.
