Why does MAC respond poorly to antibiotics at 28?

Hello doctor,

I am 28 years old. I was diagnosed with a Mycobacterium avium complex (MAC) lung infection, and honestly, it scares me more than the medications. I was told that standard antibiotic regimens only work about half the time, and that hit me hard.

I have been on multiple antibiotics with side effects, including nausea, fatigue, and constant worry about resistance. Some days the symptoms ease, then flare again, and it affects my mental health.

Why does MAC at the age of 28 have only a 50/50 chance of responding to standard antibiotic regimens?
Is it because the bacteria grow slowly or hide in the lungs?
Do drug penetration and immune response matter this much?

I am young and doing everything right, but still feel like the outcomes are a coin toss. I am struggling to stay hopeful when the success rates sound so uncertain.
