Hi doctor,

I am suffering from headaches for few years now and I see few horizontal lines appearing to be 3 to 5 mm thick. It is seen in both eyes and they are more pronounced in low light when the eyes are closed. I had undergone a retina test and results were normal. I have a set of four MRI images showing something near the ear that is bright white on T2 images and darker on T1 on the side marked R. I am curious if it is something that I should be concerned about. I have not got the report yet. Please help.

I am not sure, but if you are talking about T2 white areas in the right mastoid temporal bone, it needs to be confirmed on all the MRI (Magnetic resonance imaging) scan images. A T2 bright and T1 dark area usually shows fluid contents, and if I am correct, it can be a mild right mastoid effusion. If you feel that my reply is not about the area you want to be reviewed, please send a marked image and share the CD images to check the complete scan, and then I can help you accurately.

Hi doctor,

I do believe that you are right about the place I am referring to. I have recently had some ear discomfort, and I am not sure how slow or fast it progresses, but over the last few years, I have also had some unexplained fevers. I uploaded another image with the area circled to confirm. I truly appreciate you pointing out a potential cause for what I see in the images.

I have seen the images (attachment removed to protect the patient's identity) and hope you are not in much discomfort. A right mastoid effusion might suggest a right middle ear infection, and this needs to be investigated in detail by doing an HRCT (high resolution computed tomography) temporal bone scan. It is more informative than an MRI (Magnetic resonance imaging) brain scan and shows the small parts of the middle ear in detail. Mastoid infection might follow an acute or chronic course and needs a detailed clinical and imaging evaluation to suggest a treatment.

Please get it clinically examined by an ENT doctor, and an HRCT temporal bone scan might be recommended for you.
