Hi doctor,
My Seven year old daughter started behaving strangely for the last two months. She started feeling something that stuck in her throat and she cannot swallow. Then, she said that all foods are junk and would ask whether a particular food can be taken after another food, etc. For example, Can I have milk after chocolate or rice after fruits? She feels that her hand is dirty and does not use hands to have food. Always, mother or grandmother has to feed her. She does not like taking bath or brushing teeth while going to school. At that time, she got angry, uses abusive words and causes physical harm. When she gets angry with anyone, especially relatives and parents, she uses abusive words and causes physical harm by pinching and kicking. She spits at the person or on the floor. Her spitting occurs frequently without provocation. She touches each object she comes in contact and then places her fingers over her forehead repeatedly. She her mouth full of saliva during class hours. She is calm at other times, but gets irritated very fast. She feels guilty after causing physical harm and says sorry to mother only. We feel that she cannot control her anger. Kindly help us with the diagnosis. What is her condition? How to solve this? Can this be treated with counseling?

Your daughter seems to be having some symptoms of OCD (obsessive compulsive disorder). But, before proceeding to the diagnosis, I want to know if she had any fits in the past or any history of headache, blurred vision, vomiting, fever, etc. Has she been suffering from frequent throat infections? I suggest brain imaging to rule out an organic cause for the symptoms. Also, I suggest capsule Prodep 20 mg (Fluoxetine) once daily along with tablet Lonazep 0.25 mg (Clonazepam) at night for a period of two weeks. The child has to be interviewed in detail separately. Consult a specialist doctor, discuss with him or her and take the medicine with consent.
