Hello doctor,

I have been diagnosed with multidrug-resistant tuberculosis. I have attached the reports. Please suggest a way to treat this.

I have seen both reports (attachment removed to protect the patient's identity). Sputum report is positive and has also grown Mycobacterium tuberculosis. But the sensitivity test for all five first-line drugs is not done. Patient weight is also not mentioned. Decreased immunity might be a cause. If it is MDR-TB (Multidrug-resistant tuberculosis), these second-line drugs should be started with remaining sensitive first-line drugs. When there is no report of resistance to Isoniazid or Rifampicin, only then we say it is MDR-TB. So, please get the sensitivity test done for all first and second-line drugs. The treatment plan is anti-tubercular treatment. The patient must take an injection of Streptomycin 750 mg IM (intramuscular) daily along with tablet Akurit 4 (a combination of Isoniazid, Rifampicin, Pyrazinamide, and Ethambutol) according to weight. Also, give protein supplements like eggs and protein powder with milk. The investigations to be done include Sputum AFB (acid-fast bacilli) culture and sensitivity test for first and second-line drugs. As a preventive measure, the patient should compulsorily use a face mask. Thank you.
