Hello doctor,

I have dark blotches on my neck and under my ears that never seem to go away. Sometimes, they are slightly sensitive or itchy. The cause does not seem to be related to any fabric or detergent. I use Benzoyl peroxide face cream but that does not seem to help. It has been probably present for more than two years, but it was so mild that I did not bother about it. Please help.

From the picture that you have shared (attachments removed to protect the patient's identity), it seems that you have melasma or PIH (post-inflammatory hyperpigmentation). Melasma is an acquired pigmentation of sun-exposed areas. Areas that receive excessive sun exposure, including the cheeks, the upper lip, the chin, and the forehead, are the most common locations.

Melasma can be difficult to treat. The pigmentation in melasma develops gradually, and resolution is also gradual. I suggest you follow certain general measures which are as follows:

  1. Avoid direct sunlight.
  2. Use broad-brimmed hat if you go out in the sun.
  3. Use skin lightening cream during night hours like Clearz cream (Kojic acid).
