Hello doctor,

I am a 20-year-old female and have had irregular periods since I was 15. But they have been regular for the past year. I have done an ultrasound for PCOS multiple times on the recommendation of my doctor but found no signs of PCOS. I had my last period two months ago. I had protected sex towards the end of the last month. But I have not got my period yet. I have taken an at-home pregnancy test, which was negative. Could I be pregnant?

I understand your concern. At times, pregnancy kits can show false results. However, according to your dates, there is very less possibility of pregnancy. I suggest you repeat the urine pregnancy test with a morning sample. If it is negative, you can induce your periods by withdrawal method. You can check on the internet for more details on the method. When was the last time you had done an ultrasound scan? Have you checked your thyroid profile and complete blood count (CBC)?

Thank you for your reply.

I had taken the test with the morning sample. However, I will repeat it. I was on hormonal pills last year due to irregular periods. But my periods have been regular since last year. Hence my doctor advised me to stop my medication. I took my last USG scan last year. I have not checked my thyroid or CBC. I want to know that if my second test is also negative and I induce periods with the withdrawal method (which is through medication I suppose), will there be any side effects on my cycle in the next months or later? I also want to know if the medication used to induce periods is the same as an abortion pill. What should I do if my second test comes negative? Can you guide me to induce my periods?

I suggest you repeat the pregnancy test. If it is negative, I advise you to take tablet Medroxyprogesterone acetate 10 mg twice daily for three days. Consult your doctor and take the medicine accordingly. After stopping this tablet, you will get your period within 2 to 7 days. These are not abortion pills, these are progesterone-containing tablets, and they will not affect your future cycle or it does not have any major side effects. These are safe to take. If you will get such delayed periods in subsequent cycles, I suggest you do USG (ultrasonography) pelvis scan, thyroid profile, and CBC (complete blood count) to rule out any underlying pathology.

Should I take these tablets even after a negative test? Will it be risky to wait for my period to come naturally? I am worried that taking tablets will make my periods irregular again in the future. Also, if I take these tablets and induce periods, will it mean I am not pregnant? I had protected intercourse with a condom and followed the pull-out method to be extra cautious. Will these tablets not cause any hormonal imbalance again? I am confused about whether to wait for my period to come normally or just take these tablets after the second negative pregnancy test. Please help.

I understand your concern.

There is already a hormonal imbalance, which is why you have not got your period yet. These medicines should be taken only if the pregnancy is negative. 21 to 35 days cycle is considered normal. However, it is already 45 days, so it is better to take medicines to induce periods.
