Hello doctor,

I was confirmed pregnant a few months ago, and I have given birth twice before (six and seven years ago) through vaginal delivery. I have been actively using heroin (IV) and cocaine (smoking) for eight years on and off. I have blood clot-like discharges come out.What are the steps that I should take after a miscarriage? Is there anything else that will happen?

Spontaneous abortion, also known as miscarriage, is defined as the loss of pregnancy at less than 20 weeks of gestation. On transvaginal ultrasound, there would be no remaining products of conception in the uterus. Ultrasound is the gold standard for examining intrauterine contents and viability. I recommend abstaining from vaginal intercourse for one to two weeks after the passage of tissues. Kindly educate yourself regarding contraception after a miscarriage, including the use of an intrauterine device or hormonal contraception, as both have been deemed safe for use immediately following an early pregnancy loss. It is important to provide each RhD (Rhesus factor) negative patient education on alloimmunization and the risk of future pregnancy with regards to receiving RhD immune globulin. Please take care.
