Hi doctor,

My husband was taking the tablet Epilim for 20 years as prescribed by a neurologist. He decided to stop taking the medication two years ago. He experienced drastic changes in his behavior, mood, and sleep. He was not sleeping for at least 12 months and could not cope with stress. I think he has anxiety and PTSD. The neurologist diagnosed him with generalized epilepsy however he never had a seizure. I believe he was misdiagnosed and should have seen a psychiatrist, not a neurologist. He has no awareness that he has an issue and blames his inability to cope with stress on our relationship. He refuses to get help as he does not think he has a problem. What would be the effect of coming off a psychiatric drug like the tablet Epilim after taking it for 20 years? I am worried about his mental health.

Thank you for taking help from a psychiatrist. I can understand your situation and your concern about your husband. Your husband has taken tablet Epilim (Valproate) for 20 years. It is a mood stabilizer as well as anti convulsion. If it is stopped suddenly it will cause irritability as well as sleep disturbance. It also depends upon the dose of the medication. If he is taking a high dose and stopped suddenly then he may have withdrawal. Your husband may have an anxiety disorder (as per you). There is non-pharmacological management for him, he should try to follow the below guidelines to deal with it. It will help him.

  1. Your husband should do exercises daily for 30 to 45 minutes like at least going for a walk.
  2. He should do meditation for 20 minutes daily.
  3. He should eat healthy home-cooked foods, and avoid junk foods.
  4. He must drink enough water.
  5. He should spend quality time with friends and family daily.
  6. Your husband should spend time on creative hobbies like reading good books, writing journals, painting, drawing, and learning new skills or language or musical instruments.
  7. He should take enough seven to eight hours of sleep daily.
  8. He must avoid tobacco or alcohol or any substance abuse.
  9. He should start tablet Escitalopram 10 mg daily in the morning after breakfast and tablet Clonazepam 0.5 mg at night.
