Hello doctor,

I thought I got bit by something three days ago. I went to the hospital yesterday morning at 5:00 AM and was told it was MRSA. I had MRSA a couple of times in the past 8 years. This is the worse case I ever had. The hospital gave me two antibiotics Cephalexin 500 mg 4 times a day and Bactim DS tablets two times a day. It has been 22 hours on antibiotics. It is getting worse and I started running a fever of 100.4 tonight. The pain is increasing and I can barely walk because of the pain. I called the on call doctor and he was concerned that I was running a fever but said there is nothing we can do now. Make an appointment tomorrow and we will have to come up with a new plan to treat this. I cannot get in till tomorrow afternoon. It started with a bit looking like bump. Now it has spread out to be as round as a baseball or a little bigger. Earlier it had a black dot in the middle and now it is turning a black color around the black dot. They gave me no cream to put on it, no steroids to help with the inflammation. Just two antibiotics. I was in and out of the emergency room yesterday within 15 minutes. What should I do?

Investigations to be done

As you have developed fever, it is advisable to get a blood culture sensitivity test done. Though the report will come in 48 hours, it will help in a better choice of antibiotics. Though antibiotics can be changed to Linezolid or Vancomycin, but it is best that it should be done with the consultation of your examining doctor. Till that time you can continue with the same antibiotics you are already on.

Treatment plan

Take tablet Paracetamol 650 mg twice a day. A combination of Aceclofenac and Seratiopeptidase twice a day will help in decreasing pain and swelling associated with the disease. Apply Mupirocin cream (T-bact) over the lesion twice a day.

Preventive measures

Hot fomentation compresses will help with the inflammation.
