Hi doctor,

I have been experiencing the issue of nightfall for the past year. After four months, my semen became very watery, resembling the absence of semen. I heard that masturbating is a healthy solution, so I masturbate once a week, but nightfall still occurs. There has been no improvement, and my semen remains thin. Why is this happening?

Wet dreams or nightfall, also called as nocturnal emission, is normal. It can be due to increased erotic stimulus, whether mental, physical, or visual, without engaging in sex or masturbation. At this age, you will be experiencing good testosterone production, with approximately 20,000 sperms produced every minute, leading to a significant amount of semen production. It is important to release it through masturbation or sex; otherwise, it may occur on its own when you are very relaxed (during sleep). Knowing the cause makes treatment easier. To address the issue, consider reducing the frequency of erotic stimuli or engaging in masturbation or sex. Here are ways to reduce the frequency of erotic stimuli:

  1. Limit the intake of caffeinated beverages and foods after late afternoon, as consuming caffeine before bedtime can stimulate the brain and lead to erotic dreams.
  2. Take a cold shower before bedtime to decrease excessive sensitivity in the genital region.
  3. Limit and control stress levels, as stress can be a triggering factor for various issues. Monitoring stress levels is crucial for achieving a peaceful night's sleep.
  4. Avoid sleeping on your back or stomach, as these positions are known to cause sleep orgasms. It is better to sleep on your side.
  5. Exercise regularly to keep your body and mind relaxed. Excessive tension, anxiety, and body fatigue collectively increase the likelihood of sleep orgasms.
  6. Avoid watching and reading erotic content at night, as it stimulates both your body and mind, resulting in wet dreams during sleep.

For a temporary solution, I suggest you use tablet Pari (Paroxetine) 10 mg once a night for 15 days.
