Hello doctor,

I am 40 years old. I weigh about 81 kg and my height is 5.1 feet. I am over obese with big belly and fat arms. I eat only a little and work more. I also have constipation. Please help.

Yes, you are suffering from obesity. Are you breastfeeding?

The possible causes of weight gain and obesity are:

1. Metabolic syndrome.

2. Renal dysfunction.

3. Hypothyroidism.

4. Anemia.

5. Fatty liver disease, etc.

Do regular exercise, eat green leafy vegetables and fruits. Use tablet Pantoprazole before breakfast for one week.

Please get it done thyroid profile, complete hemoglobin status, serum creatinine, and ECG (electrocardiography).

Eat millet rice for weight reduction, use tablet Evion 400 mg twice daily for five days.

Investigations to be done

USG abdomen. Complete blood picture. Fasting lipid profile. ECG.

Differential diagnosis

Postpartum weight gain. Metabolic syndrome.
Serum creatinine.

Probable diagnosis

Obesity under evaluation.

Treatment plan

Tablet Evion-LC twice daily for two weeks.

Preventive measures

Avoid junk food and cool drinks. Avoid processed carbohydrates and non-vegetarian diet.

Regarding follow up

Review after one week.

No breastfeeding. My second daughter is 8 years now. I do not take any junk food or fast food. I eat only a little bit homemade food. Due to obesity, I am very getting upset regularly. Kindly resolve my problem.

I can understand your problem, but you have to get it done above investigations to know where the problem is. After seeing your kidney function, hemoglobin status and fat in your liver, I will advise a course of medication. Also, get it done ECG and serum creatinine also.

Everything is normal except serum cholesterol is a little elevated. So you may take tablet Orlistat 120 mg after food daily afternoon. It may cause weight loss in a period of six weeks. It may also cause loose stools in a few users. You may take a multivitamin tablet once a day one hour before or after the Orlistat tablet.

No need to take these tablet with fruits or vegetables. It will reduce the absorption of fats from your food. So your weight gets reduced. If your weight is not reduced or developed any loose stools, then you may message me.
