Hello doctor,

After having spicy food, I got an ulcer on my tongue. I consulted several doctors. But they told me it is not serious. I quit smoking four months back and never smoked for more than ten years. I was recently diagnosed with acid reflux and an allergy to dust mites. I already tried many mouth gels and multivitamins in the past few weeks. It looks like foliate papillitis. It has not increased but I felt like it has slightly reduced. There is no other symptom. I got tested for thyroid stimulating hormone where the levels are higher than normal hypothyroidism. Triiodothyronine (T3) and thyroxine (T4) levels were normal. Please suggest to me why the ulcer is not going away.

I have gone through your history and the attachment provided (the attachment removed to protect the patient's identity). It looks like a malignant ulcer such as a carcinoma tongue. The ulcer was large in the initial image and now reduced significantly and does not look like a malignant ulcer. I suggest you consult a specialist. As you smoked for ten years, so there is a chance for malignancy. So be careful regarding any ulcer in the mouth, especially, if it is at the same location.

What should I do? Do I need to wait and watch or immediately go for biopsy. But cutting the tongue for biopsy may lead to other complications. I am getting so scared and worried about it everday. Please suggest me what should I do next.

As of now, I suggest you wait and watch. There is no need for a biopsy. But if you develop a lesion again at the same site, then you should go for a biopsy. In a biopsy, they will take very small tissue from the tongue. No major complications will be seen.

Same symptoms don't mean you have the same problem. Consult a doctor now!
