Hello doctor,

I am male, 43 years old, 6 feet height, and 115 kg weight. Pain in the penis for the past three months now. Have visited two urologists. Have taken a course Zocef 500 mg for five days and applied Candid B cream for one week and taken Forcan 150 mg one tablet and again visited the same doctor and was advised Levoflox 500 mg for two weeks about two months back. I have completed the course.

The second urologist advised Azithral 500 mg for three days and Doxy for seven days. I have also completed the course about 1.5 months back. Have done self-medication of Candid B cream 10 days back for seven days along with Forcan two tablets. Some few days I have relief and some days it gets bad. There is no relief as now the pain keeps coming back. Presently, I am having pain again.

There is a possibility of non-gonococcal urethritis because the antibiotics you have taken should have cleared it if it was a routine infection. I would suggest Doxycycline 100 mg morning evening along with Veltam 0.4 bedtime for two weeks. You should start feeling better in 48 hours after starting medicine.

I have already completed the Doxycycline 100 mg for seven days morning and evening course as mentioned and also there is no problem while urinating. There is no burning sensation and the flow is also good.

Burning sensation is always due to urethritis because you have taken lots of antibiotics so there may not be an infection but only inflammation and one-week doxycycline is not enough that is why I said two weeks and added a urethral relaxant Veltam. Candid B is helpful in fungal infection of skin because it contains steroid Betamethasone. It will help for some symptomatic relief but transient. I will advise you to continue Veltam and Doxycycline, avoid dry fruits and spices, take enough water.

Thank you for explaining again with patience. Just a query, is this problem skin related? Or is it an internal issue? The reason for this problem? Do I need any other cream to be applied? Why all the above medication has not helped me till now? Is the infection or disease transferrable? Just that I am fed up of taking so many medicines and it has taken so long. I have this constant pain, irritation in my penis. Some days no issue and suddenly the pain comes back.

The urethral irritation and burning are generally more due to inflammation and less due to infection. Second, the organism can be resistant to the antibiotics already received. I believe that you have just inflammation rather than infection with is causing urethral spam that is where the role of Veltam comes to prevent inflammation of the urethra and keep it relaxed. If there is any infection, the doxycycline will take care of that, you should be fine with these medicines.

Thank you for the reply.

I am still not feeling better with the above medication. I stopped my medication a week back after the course. Still I am having pain in my penis. There is no issue with the flow of urine. After I took the above medication, I had problems ejaculating. Now it is fine. I do not have any issue with errection and sex. But after masturbating or having sex, the pain in the penis rises a little more. Could the pain be due to prolonged sitting due to my work?

I feel some pressure building on the penis while sitting. I was earlier weaing very tight jeans and now started wearing comfortable-fit trousers. I have also started evening walk for the past two days. Also, I smoke about ten cigars daily and drink alcohol about 3 to 4 times a week. As mentioned above, I have taken too many antibiotics in the past 3 to 4 months.

I suggest you continue with the tablet Veltam. You have chronic prostatitis and it needs prolonged treatment.
