Hello doctor,

I get palpitations, and I become nauseous when I go for walks. I went for a trip around three months back and got dehydrated. I became fine a few weeks back but started feeling weird due to the heat. I was puking a lot last week. I must also highlight that I did not sleep for two nights for the last two weeks. I have been sleeping on time for three days. My blood pressure is 130/90 mm of Hg. I was recently diagnosed with borderline left ventricular hypertrophy. I also have some pinching sensation on the sides of my head, especially when working on the computer. I was asked to do a fundus eye checkup and underwent a two-dimensional echocardiogram scan.

I have read about your problem and seen all the attached reports (attachment removed to protect the patient's identity). The probable cause of your problem is dehydration and electrolyte imbalance. You improved after rest and staying hydrated, but you are not taking full precautions. Your blood pressure (BP) is marginally high for the same reasons. Take adequate rest and have a soft diet. Avoid smoking or drinking alcohol. I suggest you take the following medications. Take Tablet Pantocid (Pantoprazole 40 mg) DSR once daily before food for ten days, Tablet Ambulax (Alprazolam 0.25 mg and Propranolol 20 mg) at night for ten days, Oral rehydration solution (ORS) by mouth daily, Capsule Royal Q10 (multivitamins) once daily for ten days after breakfast.

Investigations to be done

You need to undergo serum electrolytes, liver function test (LFT), and complete blood count (CBC) tests.

Preventive measures

Use mineral water and take rest.

Regarding follow up

Get back to me after five days with the reports.
