Hello doctor,

I am 45, male, experiencing burning just below the chest after eating. It has happened on and off for two years, but now it happens more frequently. Is there any remedy? My grandfather had a stomach ulcer, and I fear I may get one too. Is there any remedy?

Regarding your symptoms, I strongly believe you have developed a chronic, poorly treated stomach ulcer disease, possibly inherited from the family, which in this case is your father. The stomach ulcer is triggered after a recent meal and is worse when you eat spicy or peppered food. Have you been on any medication since it started?

Please do a urea breath test to check if a bacterium called Helicobacter pylori (H. pylori) is present, as this may be linked to no curative episode for the past two years since onset. Please take the following first-line pharmacological therapy and let me see if there will be an optimal improvement:

  • Ranitidine 300 milligrams per oral (po) two tablets, daily for two weeks.

  • Omeprazole 40 milligrams per oral (po), two capsules, twice daily (bid) for two weeks.

  • Clarithromycin 500 milligrams per oral (po), one tablet twice daily (bid) for two weeks.
