Hello doctor,
A few years back, I suddenly developed constant dyspepsia, bloating, and reflux. Also on and off stomach pain. I was prescribed Lansoprazole and it took away the symptoms until a few weeks back. Symptoms have suddenly returned despite still taking PPI. Can an ulcer develop whilst taking Lansoprazole? I am also having minor pain under the right ribcage. Can an ulcer develop or worsen whilst taking a PPI?

Peptic ulcer disease is mostly related to the presence of Helicobacter pylori infection (a bacteria that is transmitted via saliva and is present in around 70 % of adults). The first step of the treatment is actually PPI. When this stops working (yes, you may develop more serious symptoms. Whilst taking PPI's) you have some options like add antiacids whenever you have symptoms like Magaldrate-Simethicone 3 to 4 times a day. Add H1 blockers (Ranitidine 150 mg tablets) every 2 to 3 times a day. Avoid over-the-counter pain-killers (with some exceptions such as COX2 inhibitors).
If symptoms continue despite this, you need to do an H.pylori testing, if positive you need to take the medication for its eradication (it includes two antibiotics). This usually improves very much the symptoms for months or years.
