Hello doctor,

Recently I discovered a dark spot almost black in color over my nose. I had a pimple in that exact area. It was a pimple that was deep under the skin, painful to the touch. It never showed a whitehead so I didn't pop it. I left it alone and it went away on its own. Now I noticed this black spot. Do I need to worry?

I can understand your concern.

The current lesion does not appear as melanoma. It is common for acne to leave scars when they are deep under the skin which is termed as post inflammatory hyperpigmentation. They will take time to subside and reduce in intensity which may happen over time and may take up to 2 years without any treatment.

However, for a speedy recovery, you may consider medications both topical and oral along with undergoing laser treatment or chemical peels which help with the regularization of melanin pigment production.

As for confirmation as you have noticed changes in the appearance, I would also highly recommend consulting a dermatologist in person since melanoma can be ruled out through a dermatoscopy and tests which may require the doctor to inspect, touch, and feel the affected area for any other elements on the affected area.

In the meantime, I would also recommend using sunscreens for further preventing further darkening of the pigmentation.

My advice :

1. Apply Glycolic acid 6 % cream in a thin layer on the affected area at night for 2 weeks only.

2. Use a good sunscreen with SPF (sun protection factor) 60 daily and reapply it every 2 hours.

3. Take a tablet of Vitamin C twice daily for 2 months.

4. Take a capsule of Vitamin E after dinner for 2 months.

5. Make sure to avoid oily and fried food, sweets, red meat, milk and dairy products. Consume more fruits, vegetables, fiber, and at least 3 liters of water per day.

Having said that, as I had previously mentioned, it is important to rule out other possibilities through direct consultation and examination.
