Hello doctor,

I recently had my period last month. I had unprotected sex three days after my periods ended, but I took an I-pill within 12 hours. My period usually arrives around the same time each month. Now, I am two days late for my expected period. Please help.

I read your query and can understand your concern.

Ovulation usually occurs around the 12th to 16th day of the period. The life of the egg is 24 hours, and that of the sperm is 72 hours. So there was no chance of the egg being available for a possible sperm, and sperm would not be available for the next ovulation.

I-ill was not needed actually. I-pill is a high dose of Progesterone hormone, which is produced in your body in the second half of the period after ovulation. This pill will suddenly increase the thickness of the inner lining of the uterus, making it unfavorable for a possible pregnancy to sit. As it is a single dose, it has no strength to stand on, so it falls off, and you get period or bleeding of the increased thickness within seven to 10 days.

This pill is a very effective method, but it disturbs the hormonal balance, so the next period may be disturbed. Pregnancy is not the only reason for no periods but still has to be ruled out. So If I may advise-

I suggest taking a urine pregnancy test with an early morning urine sample. I am sure it will be negative. Then have any of the birth control pills (packet of 21 pills or 21 active and 7 inactive pills)- have one tablet twice a day for 10 days ( throw away the 21st pill and the inactive pills), no matter what the cause is (except pregnancy), you will get periods within seven to 10 days of the last pill. This will also regularize your hormones and periods.

Other possible causes may be a sudden increase or decrease in weight, any significant physical or mental stress, and thyroid or prolactin hormone abnormality.

So, I advise you the following investigations to be done in the early morning on an empty stomach from 6 to 9 am

1) Thyroid profile.

2) Serum Prolactin.
