Hello doctor,

I am pregnant, and my last period was three months ago. Today I underwent the first ultrasound for a fetal heartbeat, but the report shows no heartbeat.

When is the right time to go for the next ultrasound, and what should I do next?

I was diagnosed with PCOS in an ultrasound four months back, but my period has always been regular. I am on Folic acid.

Please help.

It is completely understandable to feel concerned after this ultrasound. Please take a deep breath and know that this is a common situation, and it does not automatically mean something is wrong.

Based on your last period date, you are likely around seven weeks pregnant. At this very early stage, it can sometimes be too soon to detect a heartbeat with an ultrasound, especially if your ovulation occurred a little later than usual.

This is even more possible with a history of PCOS (polycystic ovarian syndrome), as cycles can vary slightly. My best advice is not to lose hope. The most appropriate step is to wait and have a follow-up scan in seven to 10 days.

This allows more time for the pregnancy to progress to a point where a clear heartbeat should be visible. It is this second scan that will give us a definitive picture. In the meantime, please continue taking your Folic acid.

I know the wait will feel anxious, but try to be gentle with yourself. Look out for any severe pain or bleeding, and contact your doctor immediately if that occurs. Otherwise, we will let the little one have a bit more time to grow and will check again soon.
