Which prostate cancer therapy preserves erectile functions?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I was diagnosed with Gleason 3+4=7 prostate cancer last month and am considering both surgery and radiation therapy. My primary concern is maintaining sexual function, as I am currently sexually active and do not have erectile dysfunction. Which treatment generally leads to better outcomes for sexual function? Additionally, should I consider focal therapy? Please help.

Thank you in advance.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

While undergoing hormonal therapy for prostate cancer (a type of cancer that occurs in the prostate, a small gland in the male reproductive system that produces seminal fluid), one of the most effective ways to preserve sexual function is through stereotactic ablative radiation therapy (SABR) or stereotactic body radiotherapy (SBRT).

These advanced radiation techniques utilize multiple smaller, focused beams of radiation, which are directed from different angles to converge precisely at the tumor site. This allows for the delivery of a highly targeted dose of radiotherapy directly to the tumor.

The key advantage of SABR and SBRT is their ability to administer a high dose of radiation to the cancerous tissue while minimizing the exposure of surrounding healthy tissues to radiation. This precision helps protect vital structures, including the nerves and blood vessels that play a role in sexual function.

By sparing these sensitive areas, the likelihood of side effects related to sexual health, such as erectile dysfunction (inability to maintain an erection), is reduced. These techniques are particularly beneficial for patients who wish to minimize the impact of treatment on their quality of life, including preserving sexual function.

SABR and SBRT provide a promising option for prostate cancer patients who are concerned about the potential effects of radiation therapy on their sexual well-being while effectively treating the cancer.

I hope this helps.

Thank you.
