Hello doctor,

I was recently diagnosed with a heart rhythm issue and atrial fibrillation and am scheduled to have a catheter ablation in a little over two weeks. I am currently taking tablet Multaq 400 mg, one in the morning and one in the evening, tablet Diltiazem 240 mg in the morning, tablet Eliquis 5 mg, one in the morning and one in the evening, tablet Metoprolol, 25 mg once in the evening, and Simvastatin 10 mg. I was advised to have fairly regular sex but have problems urinating after sex or ejaculation. I drink a lot of water after ejaculation and have had a hot sitz bath for a while, which seems to help a little bit. I do not have real pain in the area, no fever, no blood in my urine, and no pus, which I had many years ago when I had prostatitis. Could I have prostate enlargement or a urinary tract infection?

From your history, I suppose it is prostate enlargement, and symptoms get more pronounced when you ejaculate as this sex gland enlarges further, causing urinary retention. I suggest you get ultrasound KUB (kidney, ureter, and bladder), uroflowmetry, and serum prostate specific antigen tests done. I suggest you can start tablet Tadalafil 5mg once daily two hours after a meal. This tablet will enable you to pass urine and help you achieve better erections. However, please consult a specialist doctor, discuss with them and start taking the medications with their consent.

Thank you for the reply.

Can you possibly prescribe the tablet Tadalafil? I made an appointment with a urologist here, but the earliest opening here is only after five months, so I appreciate your help and insight. I will try and get in to see someone earlier. Does a hot bath or sitz bath help, or anything else I can do to help?

I read your query and can understand your concern. Sitz bath will not help with prostate enlargement. As far as the prescription is concerned, unfortunately, I cannot as I am not practicing in your place. However, you can take Saw palmetto, a herbal supplement that does not need a prescription.
