Patient's Query
Hello doctor,
I am 43 years old, and I have been suffering from joint pains and stiffness for the last eight months, and the condition is worsening gradually. My hands and wrists are affected initially, but now shoulder joints, knee joints, and foot joints are also affected. Morning stiffness is so severe that I cannot move an inch within the first hour of my waking up.
There is swelling in my fingers along with painfulness that makes it difficult to type on computer in my office. I feel fatigued throughout the day.The rheumatologist diagnosed rheumatoid arthritis and wants to start me on Methotrexate and other medications.
I’m afraid of the side effects; they talked about possible liver damage and higher chances of infections. I have two small kids, and I’m concerned about being on immunosuppressants with them. Some days, the pain is so bad I can barely function, and I am missing work frequently.
I used to be very active, running, playing tennis, but now I can barely walk around the block. Will these medications really help?
Are there natural alternatives that might work?
Kindly suggest.
Hello,
Welcome to icliniq.com.
I know what you are worried about.
Your complaints of morning stiffness for a prolonged period, swelling in the small joints, and gradual pain are very indicative of rheumatoid arthritis. It is an autoimmune disorder where your immune system will attack the joint lining.
The reason your doctor is suggesting Methotrexate early is that RA can cause permanent joint damage within the first one to two years if not controlled. The goal is not just pain relief; it is to stop long-term damage and disability.
Methotrexate is considered the “gold standard” first treatment. Most patients tolerate it well at low weekly doses. Liver problems are possible but uncommon when blood tests are monitored regularly. You will have routine blood tests to keep you safe.
About infection risk, the doses used in RA are much lower than in cancer treatment. Yes, there is mild immune suppression, but most patients live normal lives, care for children, and work normally while on treatment. The important thing to understand is this: uncontrolled inflammation is more harmful than the medication.
Ongoing inflammation causes joint destruction, fatigue, heart risk, and disability. Natural approaches like an anti-inflammatory diet, turmeric, omega-3, exercise, and stress reduction can support health, but they do not stop immune-driven joint damage on their own. They can be used alongside medication, not instead of it.
If treated early and properly, many patients go into remission and return to active life, including running and sports. The earlier the treatment starts, the better the outcome. Your fear is completely understandable. But modern RA treatment has changed lives. The goal is to give you your normal life back, not take it away.
I hope this helps you.
Thank you.
