Patient's Query

Hi doctor,

I have been experiencing a persistent rash with bumps on my cheeks, jawline, and chin since mid of a year. Despite trying Hydrocortisone and eczema ointments or creams, the bumps have not improved. I started experiencing these breakouts shortly after discontinuing the pill in November and getting the Mirena inserted, which I recently had removed. I have now resumed taking the pill.

This breakout is unlike anything I have experienced before, as I previously only had occasional pimples around my menstrual cycle. I am currently taking combination birth control. Any advice on how to address and eliminate these bumps would be greatly appreciated. Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

Thank you for your query.

Based on the information and pictures provided it appears that you are dealing with acne vulgaris, a common skin condition that can affect both adolescents and adults. Acne vulgaris primarily involves the pilosebaceous follicles or oil glands and is commonly found on the face, chest, back, and upper arms.

In females, acne vulgaris may sometimes be associated with menstrual irregularities or polycystic ovarian syndrome (PCOS). Implementing certain dietary restrictions such as avoiding oily and spicy foods, junk food, milk products, and non-vegetarian items can be beneficial. It is also important to stay hydrated by drinking plenty of water and refraining from using hair oil, which can exacerbate forehead acne.

For treatment, you can consider using topical antibiotics such as 1% Clindamycin gel during the daytime and topical retinoids like 0.1% Adapalene gel at night daily for a few months. Additionally, taking Azithromycin 500 mg once daily for three consecutive days a week for four weeks can be effective.

Please use the topical retinoid gel sparingly as it may cause slight skin irritation. Discontinue use if any persistent irritation occurs. Use a gentle cleanser or soft soap like Cutiwash face wash or alcohol and acetone-free cleanser twice daily.

Ensure that you consult your general practitioner or a nearby dermatologist before starting any medication. It is essential to schedule a follow-up appointment after one month or as needed to monitor the progress of the treatment.

I hope my advice will be helpful. Feel free to reach out if you have any further queries.

Thank you.
