Hello doctor,

My daughter is six weeks old, and we have noticed that she may be lactose intolerant. After I eat dairy products, she develops small red bumps on her neck and face, which makes me think it might be related to something passing through my breast milk.

Please help.

Cow’s milk protein allergy is more common in infants than true lactose intolerance. It can cause skin rashes, especially on the face and neck, vomiting, blood or mucus in the stool, irritability, excessive gas, and diarrhea or constipation.

Symptoms usually appear within hours to a couple of days after the breastfeeding parent consumes dairy.

True lactose intolerance is very rare in newborns since lactose is naturally present in breast milk and usually only occurs after infections or in premature babies, with main symptoms being gas, bloating, and diarrhea, without rashes.

Newborn rashes or baby acne are also common in the first two months, appearing as small red bumps or whiteheads mainly on the face and neck, and are generally harmless, resolving on their own without dietary changes.

If you suspect Cow’s milk protein allergy, it will be helpful to note what you eat and when your baby’s rash appears or worsens.

Avoid all forms of dairy milk, cheese, butter, yogurt, and cream for two weeks and observe whether symptoms improve.

Watch for other warning signs such as fussiness after feeding, diarrhea, blood in the stool, or poor weight gain, as these suggest a true allergy rather than mild intolerance or acne.

If you remove dairy from your diet, ensure your baby receives adequate calcium and vitamin D through fortified plant milks or supplements.

Remember, if the baby’s rash spreads, worsens, or is accompanied by vomiting or bloody stools, seek an urgent appointment with a pediatric specialist.
