Hi doctor,

My maid has had a heart problem since birth. She complains of heart pain, sudden increases in her heartbeat, feeling hot, and leg pain. She currently feels very weak. A recent echocardiogram was done, and I have attached the results here.

Her echocardiogram (attachment removed to protect patient identity) shows that three heart valves have been affected: mitral regurgitation (MR), tricuspid regurgitation (TR), and aortic regurgitation (AR). Her left ventricular ejection fraction (LVEF) is 38 percent, indicating that her heart contractile function is significantly below the normal level, contributing to her health problems.

Additionally, her potassium level is high and requires immediate treatment. I suggest administering Calcium polystyrene sulphonate powder twice daily and advise her to avoid fruit juices, dal, bananas, and similar foods. Please ask her to consult her cardiologist as soon as possible.

She had rheumatic fever in childhood, which subsequently affected three of the four heart valves. The heart's capacity to pump blood has also changed significantly. The immediate concern is her high potassium level, which may lead to a sudden acute cardiac arrest. I have already outlined the treatment for this condition. If possible, please admit the patient to a hospital.
