Are there safer targeted treatments for rheumatoid arthritis?

Hello doctor,

I am 42 years old and have been suffering from rheumatoid arthritis for six years now. The condition is advancing very fast even after putting myself through different treatment regimes and therapies. My hands are seriously deformed, and I am unable to do things like opening containers, typing on keyboards, and buttoning up my clothes.

Morning stiffness lasts three to four hours, and I have constant severe pain despite taking prescription pain medications regularly. I have tried Methotrexate, Sulfasalazine, and two different biologics, but they either stopped working after a few months or caused recurrent infections.

The most recent biologic caused severe, persistent fatigue, and I developed pneumonia twice in six months, which forced us to discontinue the treatment. My rheumatologist now wants to try another TNF inhibitor, but I am extremely scared of further immune suppression after experiencing those serious infections.

X-rays reveal progressive erosion of my joints in the hands and feet, which frightens me about my impending disability. Being a hairstylist is becoming increasingly difficult because of my fatigue.

Are there any new therapies that have a better effect on my health without making me constantly vulnerable to infections? I urgently need a solution to control my inflammation without always being sick.

Fast-developing rheumatoid arthritis that fails to respond well to standard disease-modifying antirheumatic drugs (DMARDS) or biologics, combined with joint damage, severe pain, and repeated infections, is truly overwhelming.

The loss of function in your hands and the impact on your daily life and work understandably add to the stress and despair. It seems that traditional tumor necrosis factor (TNF) inhibitors and older biologics have not provided lasting relief and have left you vulnerable to serious infections, making the next steps feel frightening.

In addition to TNF inhibitors, there are novel drugs for RA known as JAK inhibitors, including Tofacitinib, Baricitinib, and Upadacitinib, which differ from TNF inhibitors in that they inhibit intracellular inflammatory pathways.

These drugs might benefit certain patients who fail to respond to biologic therapies, but despite the increased risk of infection, this can usually be prevented through appropriate monitoring and vaccinations.

Other approaches include changing to non–tumor necrosis factor (TNF) biologics, for example, interleukin-6i (IL-6) inhibitors or B–cell–directed therapy based on the particularities of your condition and your history of infections. Along with that, pain management, occupational therapy, supportive splints, and even surgery, such as joint replacement or synovectomy, can be useful in preserving your functionality and minimizing pain.

Since fatigue tends to decrease when the level of inflammation is reduced, the problem of controlling inflammation and maintaining the balance between immune suppression and infection needs to be addressed.

It is quite a difficult situation for you, but new, focused treatment modalities have brought hope for better inflammation management than old methods. In collaboration with a diverse team comprising your rheumatologist, occupational therapists, and possibly orthopedic surgeons, you can remain independent and live a higher-quality life.

You deserve relief that does not leave you constantly battling infections. Discussing these newer options with your rheumatologist, or even seeking a second opinion at a specialized rheumatoid arthritis center, may open the door to a treatment plan tailored specifically to your disease severity and infection risk.
