Patient's Query

Hello doctor,

I received a root canal treatment for a tooth two years back, and it remained untouched for two years. Subsequently, my long-standing dentist performed a filling on another tooth and adjusted two other teeth without my consent. Following this, I was provided with a temporary crown for the root canal treated tooth, and adjustments were made to the upper left teeth. Additionally, dental impressions were taken. On a subsequent visit, a permanent crown was placed on the root canal treated tooth, and a leaking metal filling on another tooth was replaced. More adjustments were made to my teeth, with the rationale that it is easier to adjust adjacent teeth rather than the crown itself. At this point, I intervened, expressing my reluctance for further alterations to my teeth. Consequently, I am now dealing with tooth sensitivity, jaw clicking, and facial pain. This situation is distressing, as my once flawless teeth now exhibit spaces. I am anxious about any further mishaps, particularly since my appearance is integral to my profession.

My primary concern is whether the removal of the crown would restore my facial appearance to its previous state, even considering the shaved cusps of the adjacent teeth. Given that my career hinges on my appearance, I fear additional complications. I am uncertain if my jaw and facial structure can ever revert to normal, or if I will continue to appear round-faced. Since my teeth were adjusted, I wonder if removing the crown would result in teeth that still look aesthetically pleasing and maintain my cheekbone structure. I am in a state of desperation, and while the dentist claims that my appearance would not be affected, I believe otherwise due to the changes I have experienced. If necessary, I can provide pictures of my teeth before and after the adjustments.

Please help.

Thank you

Hello,

Welcome to icliniq.com.

I understand your concerns.

When a tooth undergoes root canal treatment, it becomes brittle and susceptible to damage. Therefore, getting a permanent crown for a root canal treated tooth, like in your case, is essential.

Metal fillings can often leak after a certain period of time, leading to potential complications. It is positive that you had the filling on tooth replaced. Consider opting for tooth-colored fillings as they offer a more aesthetic and durable solution.

Sharp cusps may require some grinding to prevent unintended force on the opposing crown. But, this should be done cautiously and not excessively. Excessive trimming, as in your case, can lead to sensitivity and pain.

The spacing between teeth can result from various factors, including age-related changes and gum problems.

It is important to evaluate whether the current crown can be adjusted or corrected. If feasible, maintaining the same crown would be recommended. If not, a new crown might be necessary.

In the meantime, I would suggest you use toothpaste formulated for sensitivity to alleviate the discomfort.

Remember, any dental issue can be treated, and you need not worry about your appearance. To provide more precise advice, it would be helpful to know your age and have both extra oral (facial profile) and intraoral (gums and treated teeth) pictures.

Take care and do not hesitate to reach out if you have further questions.

Thank you.
