Hello doctor,

I am a 31-year-old male, and I have been struggling with severe dandruff for the past year. While I have had dandruff before and used anti-dandruff shampoo, this year it has worsened significantly. It is now affecting not only my scalp but also my face. There are patches above my upper lip, but it is particularly intense on my eyelids. I experience a burning sensation above my eyes when I wake up, and sometimes it is itchy and red, but the burning has become more intense lately.

The symptoms you are experiencing are typical of seborrheic dermatitis, a chronic condition caused by overactive sebaceous glands and sometimes aggravated by the fungus Malassezia, a normal inhabitant of the skin.

To manage this, stop applying oils and keep the affected area dry. Use Ketoconazole soap as an anti-dandruff treatment.

Here is the treatment plan:

  1. Take Itraconazole 200 mg twice daily after meals for two weeks.
  2. Take Levocetirizine 10 mg once daily at night.
  3. Take Deflazacort 30 mg once daily after breakfast for five days.
  4. Take Pantoprazole 40 mg once daily, 30 minutes before breakfast, for five days.
  5. Apply Ketoconazole shampoo twice a week to the scalp and affected areas, leaving it on for ten minutes before rinsing.

The differential diagnosis is sebopsoriasis.

Preventive measure: Use less oil.

Follow-up: After two weeks.
