Patient's Query

Hi doctor,

My brother had a seizure due to Xanax withdrawal. He has been on Xanax for about three to four months of 1 milligram twice daily. The seizure happened after 3 to 4 days after stopping the medication. The seizure lasted about 15 to20 econds. After he came out of the seizure, we gave him one dose of Keppra 250 milligrams. Since then, he has been taking Keppra for 2 days, which was prescribed by my mother. He did not go to the emergency room or see a doctor. Also, he has not been complaining of any type of head pain dizziness, or nausea. Here on my questions

  1. Should he stay on Keppra and for how long (epilepsy does not run in the family)?

  2. Will more seizures occur if Keppra is stopped?

  3. Could this be a one-time seizure occurence due to Xanax withdrawal?

I would like to know some further information about your brother's problem:

  1. When he had a seizure?

  2. What exactly happened in the seizure episode? Please describe it completely.

  3. What what he feel before and during the seizure?

  4. If any eyewitnesses were present there, what they saw happening to the patient: in his hands, legs, face, neck, or eyes?

  5. Was there any tongue bite or urine incontinence during the episode?

  6. How long time that activity last?

  7. Was he conscious during the whole incident?

  8. If he became unconscious, for how long duration, did he remain unconscious?

  9. Has he had any headache, vomiting, or weakness of any arm or leg?

  10. How much is his body weight?

  11. Has he any history of momentary body part jerking, especially in the morning?

  12. Has he had any seizures in past (in childhood or infancy)?

  13. How is he performing in his studies?

  14. For what medical problem, he was taking Alprazolam? Please tell in detail.

It Is very unusual to have a seizure after stopping Alprazolam (which is a weak anxiolytic). It will need his detailed information and evaluation to decide about the continuation of Keppra. If the patient is having true seizures, he will have more chance of recurrence of seizure without antiepileptics (such as Keppra). It is very unusual to have withdrawal seizures after stopping Xanax (Alprazolam).

MRI Brain with seizure protocol (contrast if required) EEG CBC/LFT/ KFT/ urine routine
