Hello doctor,
I am a healthy 42-year-old woman, 5 feet 5 inches tall and weighing 110 pounds, and would like to inquire about fluctuations in female hormones. I am still getting my period, though it is a little bit irregular sometimes. I am concerned about surges in my estradiol, luteinizing hormone, and follicle-stimulating hormone. My estrogen has been as low as 22 pg/mL and as high as 240 pg/mL. Can you confirm if these values fall within normal limits?

I just read your query and from the report you have attached (attachment removed to protect the patient’s identity), it appears that your hormones are variable during different times of the menstrual cycle. All these are perfectly normal rises and falls at different times of the menstrual cycle. I do not find any abnormalities there. There are changes in the levels of the pituitary hormones FSH and LH in the blood during the menstrual cycle. FSH (follicle-stimulating hormone) is released by the pituitary gland and causes an egg to start maturing in the ovary. It also stimulates the ovaries to start releasing estrogen. LH (luteinizing hormone) is also released from the pituitary gland and is stimulated when estrogen levels have reached their peak. LH causes ovulation to occur and also stimulates the ovary to produce progesterone. Estrogen levels rise from day one to peak just before day 14.
