Patient's Query

Hello doctor,

Two months back, I had protected oral sex using a condom and also indulged in some open-mouth kissing and breast sucking. A few days later, I started having a little heaviness in my penis after urinating. Out of anxiety, I did sexually transmitted infection (STI) panel tests after one month. Everything came negative except herpes simplex virus (HSV) IgG. Since then, I have been scared that I have contracted human immunodeficiency virus (HIV) and descended into madness. Please advise.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

As you have used a condom, you should feel protected. Proper usage of a condom would negate the risks for most STIs (sexually transmitted infections), luckily. Amongst STI panel tests, except for HIV (human immunodeficiency virus), most others can be taken as conclusive at four weeks after exposure. HSV (herpes simplex virus) IgG positivity only suggests past infection with herpes and that you have developed immunity against it now. For HIV, an ELISA (enzyme-linked immunosorbent assay) test at 12 weeks after the last exposure is considered to be definitive and conclusive. Although your chances of acquiring HIV are zero, you can still get yourself tested 12 weeks after the exposure for mental peace. Always use a condom, and try to stick to a monogamous relationship.

Hope this helps.

Thanks and take care.

Patient's Query

Hello doctor,

Thank you for your reply.

I checked there was no tear in the condom, but I am worried about the kissing part. Since last month I have descended into madness. Going through the internet looking for symptoms, and it seems to appear. I smoke approximately 30 cigarettes daily and have a sore throat now. Would a test after three months be conclusive?

Hi,

Welcome back to icliniq.com.

I read your query and can understand your concern.

You are in a tense state, and that is affecting your decision ability. Kissing alone does not transmit HIV (human immunodeficiency virus), so the risk is negligible. There is a vast amount of information on the internet, but it is helpful only if you use it with proper jurisdiction. Please stick to information provided only on authoritative sites. Also, a test at 12 weeks after the last exposure is accurate.

Hope this helps.

Thanks and take care.

Patient's Query

Hi doctor,

Thanks again.

I am also getting muscle spasms, not a weakness in general. Is this due to severe anxiety?

Hi,

Welcome back to icliniq.com.

I understand your query.

Once the fear of human immunodeficiency virus (HIV) sets in, the stress levels go up, and the body is also equally affected. Muscle spasms can be due to stress, apart from calcium, vitamin D, and vitamin B12 deficiencies. You could take supplements, capsule Uprise-D3 (vitamin D3) 60k once every week for six weeks, tablet Shelcal (calcium and vitamin D3) 500 milligrams once daily at night after dinner for one month, tablet Nurokind-OD (Methylcobalamine) once daily after lunch for a month. Get ELISA (enzyme-linked immuno-sorbent assay) done at 12 weeks, as advised earlier.

Hope this helps.

Regards.

Patient's Query

Hello doctor,

Thanks again for your reply.

I am better than before. However, I forgot something. During that, there was a superficial cut in my left index finger. No blood oozing, more like a paper cut. Most likely from her necklace. There was no blood in her nails. Is this a risk?

Hi,

Welcome back to icliniq.com.

I read your query and can understand your concern.

Such a cut is not a risk for human immunodeficiency virus (HIV).

Hope this helps.

Regards.

Patient's Query

Hello doctor,

Thanks.

I took a snap of the cut immediately after the incident. The girl had no fingernails or wounds on her hand. Please have a look and let me know.

Hi,

Welcome back to icliniq.com.

I understand your concern.

The injury is a very superficial minor abrasion. In such cuts, there is no exchange of any material possible. Human immunodeficiency virus (HIV) requires deep penetration to transmit. Your risk in such a case is zero. It should not be a reason to worry about. You can relax as the cut, in your case, is not a source of HIV transmission.

Hope this addresses your concern.

Kind regards.

Patient's Query

Hello doctor,

Thank you for your reply.

Since 12 weeks have elapsed, I did all the sexually transmitted infection (STI) tests. All came negative, even herpes simplex virus (HSV) 1. However, I am having reddishness in the cheekbone, and I am sending you the pictures. Please suggest.

Hi,

Welcome back to icliniq.com.

I read your query and can understand your concern.

I have gone through your attached picture (the attachment is hidden to protect the patient's identity), and the lighting is not at all good in it to appreciate clearly the findings. However, you can notice some amount of redness over the cheek. Redness of the cheekbone has got nothing to do with any sexually transmitted infection (STI), and it needs to be treated as a separate entity totally. You could use sunscreen with minimum SPF (sun protection factor) 30 daily in the morning and Venusia (aloe vera, vitamin E, and squalene) cream once daily at night over the cheeks for two weeks.

Hope this helps.

Kind regards.

Patient's Query

Hello doctor,

Thank you for your reply.

I am doing fine. It is regarding folliculitis that has developed at the back of the hair. There is no pain except when they are pressed. Please suggest medication.

Hi,

Welcome back to icliniq.com.

I understand your concern.

Folliculitis is a bacterial infection of hair that can happen when there is high humidity, excess sweat, irregular hair wash habits, excess oil application, increased blood sugars, etc. It is a very common condition that requires a course of oral antibiotics, when severe you can take a tablet of Augmentin 625 milligrams (Amoxicillin and Clavulanic acid) twice daily after food for five days and apply Fucidin cream (Fusidic acid) over the lesions twice daily.

Hope this addresses your concern.

Kind regards.

Patient's Query

Hello doctor,

Thank you.

Should I use any particular hair oil or shampoo?

Hi,

Welcome back to icliniq.com.

I read your query and can understand your concern.

Avoid hair oil application. You can use Keto SZ (Ketoconazole and Zinc pyrithione) shampoo weekly twice for a head bath.

Hope this addresses your query.

Kind regards.

Patient's Query

Hi doctor,

Thanks again.

One more thing. There are some eruptions in the forehead also. I have attached a picture. Please suggest medication.

Hi,

Welcome back to icliniq.com.

I read your query and can understand your concern.

What is the duration of lesions? Are they painful? Are they only limited to one side of the forehead? Are they fluid-filled or solid? If limited to one side and fluid-filled, we need to treat it as herpes zoster. If solid and recurrent, then you can treat it as acne.

Hope this addresses your concern.

Kind regards.

Patient's Query

Hi doctor,

Thank you again for your time; I greatly appreciate it.

The duration is for three days and is not painful unless pressed. It is solid and present on both sides. It happened earlier also and used Glocin (Clindamycin and Nicotinamide) gel.

Hi,

Welcome back to icliniq.com.

Fine. Have tablet Pan (Pantoprazole) 40 milligrams once daily before breakfast for a week, capsule Doxt-SL (Doxycycline and Lactobacillus) 100 milligrams twice daily after food for a week, Benzac AC (Benzoyl peroxide) 5 percent gel only over the pimples for two hours daily in the evening. Use foaming face wash twice daily for face wash.

Hope this helps you.

Thank you.

Patient's Query

Hi doctor,

Thank you for your prompt reply.

I hope you are doing well. I want to share with you that I have recently separated from my wife due to various incompatibilities. As a result of frustration, I started visiting massage spas again. I want to clarify that I did not engage in any kind of intercourse or oral sex, only received a handjob. However, for the last two or three days, I have been experiencing a frequent tendency to urinate and a heavy feeling at the top of my penis after passing urine. I have not had any fever or discharges during urination, just an uncomfortable hot feeling. I have experienced this before as well. Could there be something wrong with my penis? I would appreciate any suggestions for medication

Hi,

Welcome back to icliniq.com.

I understand the tough times that you are going through. Although you have been visiting massage spas, it is good to hear that there has not been any intercourse activity. Only a hand job alone is not a risk factor for HIV (human immunodeficiency virus). However, your complaints are more suggestive of a urinary tract infection. I recommend getting a complete urine examination from your early morning mid-stream urine sample to check for the same. I suggest you start yourself on tablet Amoxyclav (a combination of Amoxicillin and Clavulanic acid) 625 milligrams twice daily after food for five days and take syrup Citralka (disodium hydrogen citrate) 2 teaspoons (10 ml) diluted in 8 ounces (1 glass) of water three times daily. Consult your specialist doctor, discuss with them, and take the medicines with their consent.

I hope you are satisfied with my answer. For further queries, you can consult me at icliniq.

Thank you.

Patient's Query

Doctor as suggested I had Amoxyclav and also please find attached the urine report. However as I said earlier the uneasiness is still there. There is no pain or burning sensation during urine but afterwards a heaviness. I assure you that I did not indulge in anything other than handjob. Please advice.
Hi I hav gone thru d attached reports, and all of them are perfectly normal. There is no evidence of urinary tract infection/prostatitis. Also u hav effectively completed d antibiotic course too which clears d doubt. For ur persistent symptoms, it is advisable to consult a urologist and dermatologist doesn't intervene in managing these complaints

Same symptoms don't mean you have the same problem. Consult a doctor now!
