Hello doctor,

I have a one-year-old daughter, she has a rash all over her body and is running a fever of 102.5 currently taking Benadryl and Zantac she was previously taking amoxicillin for an ear infection took it for seven days, and then started to break out took her to the doctor they put her the Benadryl and Zantac because they believe it is an allergy to the Amoxicillin. She has been on Benadryl and Zantac for 36 hours getting it every six hours and is still breaking out on hives that itch. Kindly help.

I read your query and understand your concern. This could be related to viral infection, that has rash and fever. Although we need more details to delineate causative agents. You should mention the day of fever when she had the rash and the character of the rash and location on the body day by day. Preferably we need an image of the rash. The second possibility could be enteric fever. Third thing, it could be a virus known as Epstein Barr virus, and rash aggravated by the intake of amoxicillin is the characteristic feature of this virus. Fourth could be a rare etiology, Kawasaki disease. She also has a fever and rash. We can work up for all Ferber cases. And it is necessary as well. I suggest getting a complete blood count, ESR (erythrocyte sedimentation rate), Na, K, urine routine examination, blood culture, and Andy's urine culture. By the time you start on syrup. Cefixime (5ml =100mg) 4 ml twice a day. Stop Amoxicillin, Cefixime will take care of most of the bacterial infections. Continue Paracetamol 150 mg whenever she has a fever max four times a day.
