Hello doctor,

I have experienced an onset of a rash over the past 48 hours while on holiday in another country. It is super itchy and accompanied by an uncomfortable feeling. The condition is worse on my leg, as pictured, but it is also affecting my scalp, hands, feet, arms, legs, and buttocks. I also had a sore throat in the two days preceding the appearance of the rash. Please help.

The presence of rashes, along with a history of fever and sore throat during travel to another country, suggests a possible viral fever, likely stemming from the local viral strain. The current symptoms are indicative of multiple viral infections, including dengue, chikungunya, zika, rickettsial infections, influenza, and measles. Additionally, rare cases might involve acute HIV (human immunodeficiency virus) infection or bacterial infections such as enteric fever.

To further investigate, I suggest a set of blood tests, including CBC (complete blood count), ESR (erythrocyte sedimentation rate), CRP (c-reactive protein), malarial antigen, mono spot test (for infectious mononucleosis), rickettsial IgM, HbsAg (hepatitis b surface antigen), hepatitis-E IgM, hepatitis C (antibody test), RT-PCR for chikungunya, and influenza panel. Urine examination and throat swab are also essential.

To exclude other potential ailments, blood tests for malaria, viral hepatitis, infectious mononucleosis, and, rarely, locally acquired Ross River virus and Barmah Forest virus are advisable.

For the time being, I suggest you take the following medications:

  1. Tablet Levocetirizine-Montelukast: One tablet once daily for seven days.
  2. Tablet Paracetamol 650 mg: SOS or as an adjunct role for three days, not exceeding prescribed limits.

Kindly undergo the suggested tests, and kindly revert after completion. In the meantime, follow the prescribed medication regimen.
