I have had restless leg syndrome for years, due to which I am not able to sleep. I did not take any medication. I am feeling very sick due to sleeplessness.

Insomnia due to restless leg syndrome is very common. Kindly answer some questions for better suggestions-

  1. How did you diagnose restless leg syndrome?
  2. Have you consulted anyone?
  3. Are you suffering from any disorder like Parkinson's disease?
  4. Is the restlessness of the legs increased during daytime while you are sitting quietly?
  5. Does voluntary movement of the legs improve discomfort in the legs?
  6. Does your pain or discomfort improve with walking?

Do revert with the answers. I will advise some investigation and medication.

Thank you for your reply.

I am not suffering from Parkinson's disease. And yes, I have restless legs, and discomfort increases with voluntary movement of legs.

It is restless leg syndrome. Please do the following tests.

1. CBC (complete blood count).

2. Serum ferritin.

3. Serum creatinine.

4. Vitamin B12.

Start tablet Zapiz (Clonazepam) 0.25 mg at bedtime daily for 15 days, tablet Pramipex 0.25 mg one tablet in the morning. You might get more sleep, so in that case, you have to adjust the dose and contact me.

I have gone through your reports (the attachments removed to protect the patient's identity). Your ferritin and vitamin B12 are at the lower end of the normal range. But I am surprised that your serum creatinine is high.

So, for the same, I suggest you consult a nephrologist (kidney specialist) for further workup to identify the reason. For restless leg syndrome, along with the above prescribed medicine, take an Iron and Vitamin B12 tablet, Orofer XT (Iron and Vitamin B12 and other vitamins ) one tablet daily in the morning after lunch for 30 days.

Note: You might have black-coloured stool with this tablet, but do not worry.

As per your instructions, I am taking all the medicines. I could sleep well, and there was no RLS. But since yesterday afternoon, I have had the same issue. Moreover, I am traveling abroad in a few days for four to five months.

Please suggest any course of medicine that needs to be taken, as I can not buy those medicines there.

It is good to hear that you got relief from your symptoms. I suggest you continue some medications for at least six months. I am prescribing here. It includes:

  1. Tablet Zapiz (Clonazepam ) 0.25 mg one tablet at night for three months after food.
  2. Tablet Pramipex ER (Pramipexole) 0.5 mg one tablet in the morning for three months after food.
  3. Tablet Orofer XT (Ferrous Ascorbate and Folic Acid) in the morning after food for two months.

Follow up with me after three months. Do an investigation of serum ferritin after two months. And have you consulted a nephrologist? But if you are going abroad for four to five months, then carry these medications for five months.

Consult your physician and take the above medications with their consent.

Short temper is not a disease, but yes, it is a problem. So I suggest you follow the following:

  1. You have to practice yoga or meditation to calm your mind.
  2. You have to relax your mind frequently, whenever you are getting irritated or frustrated, take a deep breath and tell your mind, let it go.
  3. Do regular physical exercise for 45 minutes.
  4. Do not sit until you are fully tired. Keep yourself away from bed during the day.
  5. Practice pranayama or meditation, which will definitely help you.
  6. Get a proper sleep and eat a healthy diet.

Make a routine schedule and follow it. Be flexible. Do not take everything seriously and personally. Sometimes, the situation is not under your control, so let it go. Do not take much stress about what is happening because it is not in your hands. Take regular medication as prescribed by the doctor.
