Hi doctor,

I am a 50-year-old man. I am experiencing severe joint pain that has been increasingly debilitating. I suspect I might have non-radiographic spondyloarthritis, as my symptoms include persistent stiffness and discomfort primarily in my lower back and hips, along with some swelling in my knees. I would like to understand what diagnostic tests are available to confirm this condition, including any blood tests or imaging that might be necessary, even if they do not show radiographic changes. Additionally, I would appreciate any recommendations for managing my symptoms and improving my quality of life.

Some types of spondyloarthritis are not captured in X-rays but can be felt by symptoms only. I suggest the following measures to reduce the symptoms.

  1. Start physical activity with non-weight-bearing activities.

  2. Focus on the target areas and plan a routine with important exercises for painful joints.

  3. Avoid long-standing and long-sitting positions.

  4. Use hot packs for pain reduction.

  5. Gradually increase or decrease the repetitions as the body demands and modify the intensity as per requirement.

  6. Make lifestyle modifications like changes in activities, including basic physical activities with two hourly breaks from work every day.

  7. Do normal physical activities like stretches of the upper body, trunk, and lower body.

  8. Avoid activities like squats, forward bending, backward bending, unsupported twisting, etc.

  9. Eating a balanced diet with all kinds of fruits and vegetables in meals with the inclusion of joint-friendly foods like salmon, tuna, milk, bananas, drumsticks, and green veggies.
