I have sore breasts with itching and acne. What could it be?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 43. I have severe breast soreness and tenderness after periods. I also have severe itching on my skin, blackheads below the breast line, and folds. I cannot wear a proper bra due to itching.

I also have a bump below the left breast under the skin. What causes this?

Please help.

Hello,

Welcome to icliniq.com.

I understand your concern.

Severe breast soreness and tenderness after periods at the age of 43 is most commonly due to hormonal changes, especially perimenopause, where fluctuating estrogen and progesterone can cause cyclical mastalgia and breast heaviness.

The intense itching, darkened skin, blackheads, and involvement of the breast folds strongly suggest intertrigo with a secondary fungal infection, which is common in warm, moist areas and worsens with sweating and friction from bras. Chronic irritation can also lead to skin hyperpigmentation and clogged pores.

The bump under the skin below the left breast may be a sebaceous cyst, blocked sweat gland, lipoma, or an inflamed follicle, but any new or persistent lump at this age must be properly examined to rule out breast pathology.

The combination of pain related to cycles points toward hormonal causes, while the skin symptoms point toward a dermatologic infection or chronic irritation.

You should see a doctor for a breast examination and possibly an ultrasound or mammogram, and a skin examination, as you may need antifungal treatment, proper skin care, and guidance on supportive, breathable bras.

If the lump is painful, growing, hard, fixed, or associated with nipple changes or discharge, seek medical attention urgently.

I hope this helps you.

Thank you.
