How serious is triple vessel disease in a smoker?

Hello doctor,

I am a 42-year-old male. I smoke 30 to 40 cigarettes per day. I was admitted to the hospital last week with chest pain and underwent angiography, which revealed triple vessel disease. The ECG (electrocardiogram) showed a normal rhythm with no evidence of acute ischemic changes. Troponin levels were mildly elevated. My total cholesterol was 5.7 mmol/L, and LDL was 3.8 mmol/L. I was discharged with medications (please see the attached pictures). My general practitioner advised me to stay at home for at least a week.

My main question is how serious my angiography results are. I was informed by my doctor that all three major vessels are affected. Does this require surgery, or can I manage with medication alone? Can I continue working? I was advised to stop smoking, but I find it difficult to quit immediately. Is using an e-cigarette a good substitute? How long can I expect to live with this condition?
