Hello doctor,

I received oral sex from a CSW approximately a year ago and passed all of the STDs tests. I have not had any other activities outside my relationship. I have noticed a bump in my foreskin and have been concerned it was probably eight or nine months after the encounter.

I hope you are keeping safe. I have seen the attached photo (attachment removed to protect patient identity) of the lesion. The picture looks like Tyson's glands, which are normal sebaceous glands hyperactivity and generally do not require any treatment. However, since there is a history of risky sexual activity, if there is, presence of itching, pain during intercourse or micturition, increase in size of the lesion, burning sensation, ulcerations or blisters, the occurrence of similar new lesions. We have to rule out the occurrence of viral warts. If it is too bothersome, you can consult a dermatologist who would examine the lesion and the surrounding skin under the dermoscopy (skin microscope) to confirm the diagnosis and give treatment if it is a wart. As you have done the STD (sexually transmitted disease) panel testing, there is no risk unless you have had further similar risky sexual exposures. There is no treatment required from the pictures, but confirmation with a local dermatologist (for physical examination) is advised. I hope your query is cleared. Thank you and take care.

There has been no pain, itching, or burning, no other similar new lesions; it has remained the same for the past four months. I have been paranoid and checking for any signs. Its location also makes it hard to see, so I am not sure how long it has been there. I do not know if that changes any thoughts. Should I see a dermatologist as soon as possible if it is a wart, or will it go away on its own?

There is no need to panic as even if it is a warty lesion, it goes away with two to four weeks of treatment with Podowart (Podophyllum resin, Benzoin, and Aloe vera) solution. But it needs to be confirmed if this medication (an acid) used on normal skin will lead to skin erosion. From the picture, the white spot looks to be just a harmless sebaceous gland growth, but since you have had a risky sexual exposure, you can rule out a warty growth. I hope this helps.
