Hello doctor,

I have fever for the past two days (100 -101 degrees Fahrenheit) with urine infection and frequent urination, which decreased now. Please check my reports.

I have seen the report (attachment removed to protect patient identity). Urinary infection and fever is a condition that needs urgent attention. Poor urinary stream (difficulty passing urine) and high volume of residual urine can lead to urinary infection and fever is caused by bacteria translocation to blood. Causes can be due to enlarged prostate or urethral stricture and in rare cases neurological issue. As per urine culture results it showed E. coli infection which is sensitive to Ciprofloxacin. Fever is a condition which needs hospital admission and administration of injectable antibiotics in addition to intravenous hydration, insertion of urinary catheter, sending blood cultures, and monitoring vital signs and urine output every hour. It is risky to manage at home. After settling of infection, evaluation and treatment for enlarged prostate or causes of difficulty passing urine. In case you cannot go to the hospital then can start oral Ciprofloxacin 500 mg twice daily and oral hydration. Investigations required at the moment are ultrasound KUB (kidney, ureter and bladder) and prostate. If fever persists inspite of oral antibiotics, then needs to go hospital for admission. Follow up after ultrasound is required as we might start medications of enlarged prostate if it is the cause.

He is taking Levofloxacin 750 mg once daily for the past three days. Still he has fever around 100 degrees Fahrenheit, and it decreases after taking Paracetamol. Should we start intravenous antibiotics? And which antibiotic should he take?

I prefer to admit him to the hospital and ask for management. He needs a urinary catheter, intravenous fluids, and injectable antibiotics. According to his urine report, I prefer Ceftazidime (Fortum) 1 gm twice daily for seven to 10 days and repeating urine culture after 48 hours of starting the antibiotics.
