Could nutrient deficiency cause vomiting and thumb curling?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

The patient is a 35-year-old male, approximately 5 feet 11 inches to 6 feet tall, with an estimated weight of approximately 176.37 pounds (lbs).

He has been experiencing severe vomiting along with a reduced or altered sense of taste, making it difficult to enjoy or distinguish different foods. Additional symptoms include numbness and tingling sensations throughout the body, involuntary curling of the thumbs, generalized weakness, stomach pain, episodes of rapid heartbeat (sometimes occurring without an obvious cause), and noticeable phlegm production. He also reports cold sweats, occasional fevers, and body aches.

These symptoms tend to worsen or occur primarily when temperatures exceed approximately 30–34°C.

The patient reports having had very limited sun exposure over the past 10 years and acknowledges following a highly restricted and nutritionally imbalanced diet. His diet mainly consists of fried chicken, potatoes, rice-based meals, wheat bread/roti, and occasional consumption of lemons, tomatoes, cucumber, coriander, and mint, typically in chutney form.

Please help.

Thank you for your guidance.

Hi,

Welcome to icliniq.com

Thank you so much for sharing your concern with me.

The sole cause of the combination of recurrent vomiting, weakness, numbness and tingling, reduced sense of taste, muscle spasms causing the thumbs to curl inward, rapid pulse, cold sweats, abdominal pain, and worsening symptoms during warmer weather should not be identified as heat exposure.

Given the history of extremely limited sunlight exposure for nearly 10 years, along with a highly restricted and nutritionally unbalanced diet, significant nutritional deficiencies are a major concern. Deficiencies in Vitamin D, Vitamin B12, magnesium, calcium, and other essential nutrients can contribute to many of the symptoms described. The reported improvement after consuming fruits or supplements such as Surbex-Z may further support this possibility.

A comprehensive medical evaluation is strongly advised. Suggestions for investigations include:

A complete blood count (CBC).

The level of blood sugar.

Evaluation of kidney function.

Tests of liver function.

Panel of electrolytes, which includes sodium, potassium, calcium, and magnesium.

Electrolyte panel (consisting of sodium, potassium, calcium, and magnesium).

The tingling sensation, the curling of the thumb, the overall numbness, and the periods of rapid heartbeat are very relevant signs. These could be symptoms of an electrolyte imbalance, such as low calcium or magnesium.

Because these symptoms have been recurring over a long period and involve both neurological and gastrointestinal issues, it would not be advisable to rely solely on over-the-counter medications or supplements without proper medical assessment.

Has the patient previously undergone any blood tests for Vitamin D, Vitamin B12, calcium, magnesium, or thyroid function?

I hope this answers your questions.

Please feel free to follow up whenever needed.

Thank you.
