Hello doctor,

I am a 41-year-old male suffering from fatigue and weakness. Even a little exercise makes me short of breath. My father was a heart patient with a triple bypass. I want to find out if my weakness is due to a heart problem. I have had an ECG and 2D echo done. Both were normal. Are there any other tests that can be done to find out if the heart is healthy and functioning properly? Please advise.

Currently, I am taking Eltroxin 100 for hypothyroidism, Nexpro, and Domstal for acidity.

I go through your concern and would explain that as your cardiac tests (ECG and echo) have resulted in normal at rest (attachment removed to protect patient identity), an obvious cardiac disorder is primarily excluded. As you have coronary risk factors like strong hereditary for CAD (coronary artery disease), and probably hyperlipidemia (in the settings of hypothyroidism) and a suspicious symptomatology, an exercise cardiac stress test could be an option for further investigating any possible cardiac implication. Nevertheless, most likely factors responsible for your weakness seem to be outside your heart. First, you should clarify whether your actual thyroid therapy is appropriate and no dose adjustment is necessary. In this regard, blood thyroid hormone levels should be checked. Another issue to clarify would be the investigation of other possible influencing factors like anaemia (check a complete blood count), pulmonary function (pulmonary function tests), blood glucose level, blood electrolytes level, and blood pressure values. You should discuss it with your doctor on the above-mentioned issues.
