I have reviewed the attachments (attachment removed to protect the patient's identity). It might be caused definitely because of friction between tooth surfaces. But in my perspective as an oral surgeon, it is called Fordyce granules. It is certainly not severe or precancerous. Did you enclose another picture? Your teeth surfaces are sharp, so it might be due to the friction the oral tissues come into contact with, leading to the formation of such ulcers. I need to prescribe you an antioxidant, vitamins, and a Chlorhexidine mouth rinse regularly. Can you please reconfirm you do not have any medical issues such as diabetes, hypertension or thyroid, or other issues? Try re-enclosing the first image. I cannot open it.

I understand your concern.

I have reviewed the attachments (attachment removed to protect the patient's identity). I can see the lesion on the end of the cheek. Yes, it appears to be extensive. I was focusing only on the yellowish granules in the previous picture. There is nothing to panic about. You continue with the Chlorhexidine mouthwash as usual, which I have suggested. Do not intake any overly hot, hard, or too spicy foods. Any stimulus should not irritate this area until the white lesions vanish entirely. Ideally, I would advise you to do a biopsy to determine whether this extensive white plaque on your mucosa is possibly precancerous. Is there any history of smoking, alcoholism, tobacco, or anything? A biopsy is needed to confirm the type of cells in the mouth. There is nothing to panic about because not all lesions are precancerous or cancerous. The majority of them are just inflammatory. But this needs confirmation through biopsy, and you can get the biopsy done at an oral surgeon's clinic quickly. You can ask me anytime if you have doubts about the biopsy report or consult a surgeon.

I understand your concern.

I advised a biopsy for a white plaque lesion extending along the oral mucosa. It can be oral leukoplakia or an oral white inflammatory lesion of the buccal mucosa. I suggest you use Chlorhexidine gluconate mouthwash every night for three months, rinse 2 ml to 5 ml and spit, and take tablet Evion (Vitamin E) 400 mg tablet for 15 days every night. Differential diagnosis is oral lichen planus or leukoplakia. The probable diagnosis is an oral inflammatory lesion.
