Hai doctor,

My girlfriend and I had unprotected sex a week before her expected period. Considering her 21-day cycle, we took an emergency contraceptive pill (I-pill) approximately 6 hours after intercourse. It is important to note that she has polycystic ovary syndrome. However, she is now two days late on her period, causing us to worry. Can you please provide any suggestions or advice?

Your history is confusing. If she has polycystic ovary syndrome (PCOS), her periods are usually delayed, not coming earlier than expected (the normal frequency of periods is 28 days). If she has periods every 21 days, her expected period should have been on the 13th day of the month.

The emergency contraceptive pill (I-Pill) is a high dose of progestin hormone. It works by increasing the thickness of the inner lining of the uterus, making it unfavorable for a possible pregnancy. As it is a single dose, it is not strong enough to sustain, and the lining falls off, causing bleeding or a period of increased thickness within seven to ten days. Although you took the I-Pill on time, and it is generally effective, it can fail in certain menstrual cycle phases.

In the case of PCOS, it could be a reason for the delay in her period. These hormone pills can alter the hormonal balance and disrupt the menstrual cycle. I suggest you undergo a home urine pregnancy test using an early morning urine sample. Once confirmed as negative (observing only one line), you can consider taking any of the following oral combined contraceptive pills for ten days:

Tablet Novelone (Ethinyl Estradiol 0.035 mg + Norgestrel 0.5 mg)

Ovral-G (Ethinyl Estradiol 0.05 mg + Levonorgestrel 0.5 mg)

Duoluton-L (Ethinyl estradiol 0.03 mg + Ethisterone 5 mg)

Dear 21 (Ethinyl Estradiol 0.03 mg + Drospirenone 3 mg)

Intimacy plus 3 (Ethinyl estradiol 0.03 mg + Desogestrel 0.15 mg)

Take one tablet twice a day, discarding the 21st pill. Regardless of the cause (except pregnancy), your girlfriend should get her period within seven to ten days. Kindly consult a specialist, talk with them, and take the medications with their consent.
