I have gone through the picture (attachment removed to protect patient identity). You have scrotal dermatitis. It is a kind of allergy in which the scrotum develops a pink rash with dryness and there is severe itching. You can take tablet Levocetirizine 5 mg daily at night for ten days. Otherwise, you can take tablet Allegra 180 mg (Fexofenadine) daily at night. You can apply Hydrocortisone one percent in a very small quantity on the itchy area daily at night for fifteen days. Apply a good moisturizer at least three to four times daily. Aveeno Moisturizer (Glycerin with Dimethicone) is a good moisturizer.

There is an aggravation of itching at night time in scabies. You can have a differential diagnosis of scrotal dermatitis and genital scabies. Try to follow the treatment for scrotal dermatitis and you can use Permite BB bar soap for bathing for a month. You can apply five percent Permite cream (Permethrin) at night only on the genitals and wash it off after eight to twelve hours the next morning. Repeat the application for seven days. Maintain proper genital hygiene. Avoid scratching or rubbing the lesions. It can cause secondary bacterial infection. You can use Fucidin cream (Fusidic acid) twice daily for seven days if you get a secondary bacterial infection. Wear loose comfortable thin cotton inners. Avoid using tight clothes or inners. Avoid occlusion or friction. Avoid having sweating over there. Stay in a well ventilated cool room or environment. Wash your clothes separately. Try to wash your clothes in hot water. You need to iron the clothes. Change bed linens or pillow covers frequently. Use a separate towel and soap for you. Do not share your personal belongings. Your family members need to be treated if they have similar complaints. Maintain proper hygiene as hygiene plays an important role here. Try to add neem leaves or add 3 ml of antiseptic liquid-like Dettol or Savlon in the bathing water. Have a bath twice daily. Use lukewarm water for bathing. Have a clean and dry area.

Scrotal dermatitis is the diagnosis as explained above. But sometimes dermatitis can present with scabies along with it. As you have aggravation of itching at night time, and so treatment is need for curing both the conditions. Have a follow up after two weeks.
