Hello doctor,
I have been suffering from two major problems for the past three years. One is painful acne, scars, and blemishes; the other is sticky, oily dandruff, and hair fall for a year. The scalp flakes do not get dislodged even after shampooing. I have to dig my comb into the scalp to remove the flakes which lead to bleeding in certain areas and form crusts, and again, flakes come back in a few days. I have opted for four sessions of glycolic peels; however, there is not much difference. I also had a few sittings of Nd: YAG laser hair removal, as my doctor said that the black spots on my nose are hair. I have used tablet Isotroin 10 mg, Benzoyl Peroxide 2.5 % and Saslic DS foaming face wash for my face.
For my scalp, I have used Ketoconazole lotion, Salicylic Acid lotion, Coal tar and Zinc Pirithone shampoo, corticosteroid lotions, Calcipotriol solutions, Betamethasone and Zinc Sulphate lotions, and tablets Teczine and Itraconazole. However, I stopped all medications six months ago. I have been through several treatments that made my skin dull.
I keep my face and hair very clean. I shampoo my hair on alternate days, change pillow covers frequently, wash my face twice a day, and apply sunscreen when I go out. I work out every day and follow a clean diet barring oily foods. Yet my face is oily, dull, and full of blemishes and painful pimples. I want to remove my acne blemishes and revive my dull face, as I am starting to look older than my age. I also want to control oily hair and flaking. Please help me, as this is taking a toll on my confidence.

Thank you for the query. I suggest you stop using capsule Isotroin and start with the below treatment. Do not use hair oil and any sticky cream on the face. Avoid oily foods, chocolates, and milk products. Avoid dust, and use sunscreen (above 30 SPF). Do not rub any cream on the face. Do not use any steroid creams. Facewashes and soaps are not much helpful. Avoid touching or pressing the acne.
Use Clindamycin gel. This is more of preventive and curative treatment. You can use it all over the face in the morning. Adapalene gel to be applied over the acne at night. Take Tablet Doxycycline 00 mg once a day, Tablet Ketoconazole once daily and any multivitamin tablet for hair once daily. Consult a specialist doctor, talk to him or her, and start the medicines with their consent.

Thank you for the response.
Do I have to apply Adapalene gel all over the face or only on the acne? How do I remove the blemishes or discoloration on my chin? Also, my dandruff flakes are not dislodged even after shampooing. The only way is to pick through the comb. How do I remove them? The pictures that I have attached show the amount of flaking.
Can you suggest me a good sunscreen? How to handle painful acne bumps when I have an occasion to attend since it takes time to heal? My main concern is to control and heal acne and revive my dull skin. My hair is oily, which makes it look limp. Please advise.

You can use shampoo containing Ketoconazole and Salicylic acid twice for dandruff flakes. Cream with Adapalene and Clindamycin has to be applied only on the acne. Use any soft lotion with SPF of 50 or any good sunscreen. Also, the discoloration of the skin is due to sun exposure which will gradually reduce.
