Hello doctor,

I have a predisposition for body acne and it got worse after weightlifting so, I stopped.

I was thinking of going back but I did not since I was worried that it would get worse.. What should I do? Kindly help.

Acne vulgaris is a common skin problem in adolescents. In adulthood, it involves the pilosebaceous follicles or oil glands and is therefore seen to involve areas where oil glands are dense like the face, chest, back, and upper arms which are the common sites of involvement. You have to do certain dietary restrictions like not taking oily or spicy foods, junk foods, milk products, and nonvegetarian foods. Drink lots of water as it will keep your skin hydrated.

I suggest you do the following:

  1. Start with the tablet Azithromycin 500 mg once daily for three consecutive days in a week for three weeks.

  2. Along with this, take a capsule of Isotretinoin 20 mg once daily and apply Benzoyl peroxide 10 % gel on the pimples twice daily for six weeks.

  3. You should use 5 % anhydrous benzoyl peroxide (Perobar) soap for bathing.

  4. Besides this keep your back dry.

  5. Follow some skin care tips such as avoiding the use of cream and moisturizer, avoiding wearing tight clothes, and reducing the intake of sugary foods.

Before starting the oral medication get the following tests done.

  1. Liver function tests

  2. Lipid profile.

If these are normal, you can start with capsule Isotretinoin and revert back after six weeks as acne is a chronic disease and will take time to resolve.
