Hello doctor,

It started four years back that a patch of hair (round with about 1.18 inches diameter) suddenly disappeared from my lower right part of beard area (neck). I ignored this at first, but then when the hair never came back even after about six months. A doctor told me to use Tacrolimus 0.03 % for some time. I used it for two months regularly with not much success. Again, I ignored my condition, considering it to be a minor issue.

A month back, I moved to a new place for work and on the very second day, I lost a patch of scalp hair (left center of my head). This seems to be a very serious issue. I started applying Tacroz 0.03 % for last three weeks and am also taking Dexamethasone 5 mcg once a day. I want the hair to regrow on my beard and also on my scalp. Also, I do not want this situation or issue to re-occur.

I have noted your concern and I have viewed the images (attachment removed to protect patient identity). You seem to have alopecia areata. I suggest you take oral corticosteroids for a few weeks if it is progressing. Tacrolimus is not a treatment of choice for alopecia areata.

Can you suggest the ointment (number of times a day for both scalp and beard area)? And also the specific tablet (dosage, number of times a day and also other details such as to take on an empty stomach, after food).

For the patches use Elocon lotion (Mometasone Furoate) twice daily, Pantacid (Pantoprazole) 40 mg empty stomach and tablet Zempred (Methylprednisolone) 32 mg after breakfast. Tablets to be taken only on two consecutive days in a week.

I will take them. Shall I try this for three months before I get back to you or some shorter duration? I will apply Elocon every morning and then again at night. I will shampoo my hair every day (or almost every alternate day). I will also apply hair oil every two days in a week. Can you suggest a good shampoo and good hair oil to promote overall hair growth, hair strength, considering the circumstances?

You got it right. Use it for 4 to 6 weeks and then get back to me. Avoid hair oil while you are using lotion. Elocon lotion contains Mometasone Furoate 0.1 % and tablet Pantaocid is Pantoprazole whereas tablet Zempred is Methylprednisolone.

At least can you suggest some shampoo of medicinal value? Like a combination of treatment of dandruff, conditioner, hair growth and maybe just some help in the treatment of the condition.

I suggest you use a Ketoconazole based shampoo, daily or on alternate days. Alopecia areata can relapse after seemingly complete cure but it is not a rule. If it does relapse then it has to be managed accordingly. The treatment that I have suggested is approved for widespread patches or active alopecia areata wherein new patches are developing. Take care.
