Hello doctor,

I had been put on Primolut N for 15 days for PCOD. I started the pills on last month and finished in 15 days. On the seventh day, I had sex with my husband. I got my period on the 21st day and had been asked to begin Diane 35 for 21 days on the fifth day of the cycle. I began taking Diane 35 on 25th of last month and the pack finished on 15th of next month. Today is the last day of the pill-free week but there is no bleeding yet. I had taken an emergency pill within an hour after the intercourse. There has been no sex after the last cycle. Should I worry about an unwanted pregnancy? Please help.

As you are on hormones externally induced for internal hormonal imbalance so having bleeding according to these hormones effects on the body. Post emergency pills and a Primolut withdrawal bleed you got already then you started another hormonal pill that also acts as combined oral contraceptives pills. After finishing these tablets cyclically you will have withdrawal bleed as now menses are controlled by externally induced hormones. Have a healthy diet, daily exercise, normal BMI, and recreational activities.

For safe side, better to have serum beta hCG for your satisfaction to clear pregnancy status. Primolut which day of cycle started and you had sex before that? But you got periods after that so no worry.

I cannot tell which day of the cycle I started Primolut N. Because of the PCOD it is difficult to keep track. My cycle was rather messed up before taking Primolut. I began Primolut six days before the sex and continued taking it for eight days after the sex. I had been asked by my doctor to take it for 15 days which I did. Yes, I also had my period after the sex and I have had no sex since my last period.

You had Primolut N and sex in the same week and had emergency pill as well as per your history. After sex, Primolut N and emergency pill you got periods or withdrawal bleed. So nothing to worry. After bleed, you are continuously on oral combined contraceptive pills that are going to finish and then you will have the bleed.

I think I should wait until tomorrow for the bleeding to start although today was the seventh day of the pill-free week. I hope the bleeding begins quickly, now that the pill-free week too is over.

You are subsequently serially taking externally induced hormones that are controlling the menstrual cycle. Bleeding might be in 1 to 2 weeks of the pill-free period. In case you feel some fullness or pregnancy symptoms as this could happen when periods about to start you can have serum beta hCG to check your pregnancy status if you had sex after withdrawal bleed. Wait for bleeding to happen. If bleeding does not occur naturally and serum beta hCG comes negative you might need menstrual induction but after few days not now. Menstrual induction can be done by taking Primolut N 10 mg daily twice for a week then again wait for 1 to 2 weeks. But now no need as the system already occupied by externally induced hormones and under these effects. So let body come out if it naturally. If feel anxious can do serum beta hCG for your satisfaction if having a clear history of unprotected sex after withdrawal bleed.

Yes, better to wait for periods rather than again taking hormones now. If bleeding does not start at the end of next week then better contact here or local. You might need again hormones for menstrual induction as I described before.
