Hi doctor,

I have taken Doxepin 75 mg, but there is no improvement in my condition. But there is some relief if I take it with Amitriptyline 100 mg. Moreover, Doxepin is rarely available in my area. Kindly help.

If you have tolerated tablet Doxepin (antidepressive and anti-anxiety drug) until 75 mg, I recommend increasing it to 150 mg at night as it is one of the most sedating anti-depressants. I also recommend getting your ECG (electrocardiogram) done if you will use tablet Amitriptyline (tricyclic antidepressant) along with it. I do not remember much of your history, but you suffer from severe insomnia and headaches, if I am not wrong.

If the availability of tablet Doxepin is an issue, then I suggest the following treatment.

1. Tablet Mirtazapine (antidepressive) 15 mg at night for sleep. Do not take tablet Amitriptyline or tablet Doxepin along with it.

Or Tablet Quetiapine (anipsychotic) 25 to 50 mg for sleep.

2. Tablet Pregabalin (anticonvulsant, anxiolytics, and analgesic) 75 mg or tablet Gabapentin 300 mg (anticonvulsant) for sleep.

If you are taking one of the above for sleep, you can try adding one of the following medicines for headache prevention after taking any of the above medicines for five to seven days atleast.

1. Tablet Divalproex sodium 500 to 750 mg daily. Starting dose is 250 mg.

2. Tablet Topiramate 25 to 50 mg. Starting dose is 25 mg.

I advise you to start only one medicine at a time at starting dose under the clinical supervision of your local doctor. Do not start two new drugs together.

For dry mouth, you can try sugar-free chewing gum or sugar-free candy. These stimulate saliva production, which prevents dry mouth.

Yes, I think that would be right. You should get a sleep study done. You will get an accurate record of how much you sleep, what percentage of REM (rapid eye movement) or NREM (non-rapid eye movement sleep), and deep sleep you get. It will also rule out apnoea.

The tablet Diazepam (Benzodiazipine) is not recommended for sleep anymore. I suggest you to try tablet Lorazepam (Benzodiazipine) 1 to 3 mg instead. You can start with 1 mg and you can go upto 3 mg two hours before sleep time.

I would like to know whether you tried tablets Mirtazapine (antidepressant) or Quetiapine (antipsychotic drug) and how was your experience with it.

Nortriptyline (antidepressant) is a derivative of Amitriptyline and will cause less dryness in the mouth.
